Peer Support Specialist / Consumer Affairs – Transitions to Community Living In-Reach Peer Specialist

Trillium Health Resources is a local government managed care organization looking for Consumer Affairs – Transitions to Community Living In-Reach Peer Specialist.

Job Description: The Transitions to Community Living In-Reach Peer Support Specialist position is responsible for providing In-Reach to individuals identified in the Department of Justice (DOJ) Settlement special needs population residing in Adult Care Homes and State Psychiatric Hospitals. In-Reach is an engagement process that provides education and support to fully inform adults who have a serious mental illness (SMI) or a serious and persistent mental illness (SPMI) about community-based mental health services and supported housing options.
Education: High School/GED
Experience: Minimum 2 years’ experience working with individuals who have serious mental illness, substance use disorders, and/or intellectual/developmental disorders.
OR equivalent combination of higher education and experience.
NC Certified Peer Support Specialist Certification required.
Prefer experience with Microsoft Office programs including Microsoft Excel
Must have a valid driver’s license.
Salary Range: $25,767 - $46,313
Hours: 8:30 – 5:00, Monday through Friday are normal working hours
