Community Health Worker - FIT Wellness

Position Summary

The FIT Program Community Health Worker will provide outreach, counseling, navigational, and case management services to patients recently released from prison into Wake County with a diagnosis of a serious mental illness (for example, schizophrenia, schizoaffective disorder, or bipolar disorder) in addition to chronic medical conditions and/or substance use disorders who are referred by prison health providers prior to prison release. Patients will receive psychiatric and primary care services at WakeBrook.

Minimum Education and Experience Requirements:

Qualified Professional MH/SA

QP Requirement: Qualified Professional definitions as follows:

"Qualified professional (QP)" means, within the behavioral health system of care:

(a) an individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in MH/DD/SA with the population served; or

Certified Peer Support Specialist

Status: Non-exempt

Reports to: Team Leader, Clinical Director, Program Coordinator/Director

POM Peers on the Move CPSS

Purpose/Role

The CPSS provides peer support and resource navigation to engaging participants. This person uses their personal lived experiences to connect with participants. As someone who has successfully managed their own recovery, the PSS provides an example to program participants of what they can strive for in their own wellness. The purpose of this role is to connect with participants in areas where connections to peer support, resource navigation or harm reduction services are less accessible.

Peer Support Specialist

Genesis Project 1 works closely with communities and families to provide non-clinical support and guidance to meet the changing medical and behavioral healthcare needs of individuals struggling with mental health diagnoses or substance abuse.

We seek people who are well established in their recovery from mental health or substance abuse and have their certification as a Peer Support Specialist.
