Position Summary
The FIT Program Community Health Worker will provide outreach, counseling, navigational, and case management services to patients recently released from prison into Wake County with a diagnosis of a serious mental illness (for example, schizophrenia, schizoaffective disorder, or bipolar disorder) in addition to chronic medical conditions and/or substance use disorders who are referred by prison health providers prior to prison release. Patients will receive psychiatric and primary care services at WakeBrook.

QP Requirement: Qualified Professional definitions as follows:
"Qualified professional (QP)" means, within the behavioral health system of care:
(a) an individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in MH/DD/SA with the population served; or

Overall Function: Carries out the services of the IPS Supported Employment program by assisting clients to obtain and maintain employment that is consistent with their vocational goals. Provides support for schooling and technical training for career advancement.

Purpose/Role
The CPSS provides peer support and resource navigation to engaging participants. This person uses their personal lived experiences to connect with participants. As someone who has successfully managed their own recovery, the PSS provides an example to program participants of what they can strive for in their own wellness.
