Connecting Hearts Home Care is seeking a Full-Time and Part- Time Certified Peer Support Specialist. We are looking for a dedicated and compassionate individual that would like to make a meaningful impact in their community along with each client. 

Key Responsibilities

Individualized Support

  • Assist clients in identifying personal recovery goals and developing " Patient Centered Plans" (PCP).

Advocacy

  • Act as an advocate for clients, ensuring their voices are heard and respected by the treatment team. 

Skill Building

  • Model effective coping and self help techniques to help clients manage symptoms and increase independence

Resource Connection

  • Help clients find and navigate community resources, including social activities, housing, and healthcare.

Group Facilitation 

  • Lead or assist in peer support groups and recovery dialogues.

Qualifications

Certification: Must hold a valid State Peer Support Specialist Certification.

Education: High school diploma or GED equivalent. 

Skills: Strong interpersonal and communication skills, basic computer literacy for documentation, and the ability to work within a team. 

Other: Valid Drivers License, reliable transportation, and the ability to pass a background check. 

 

Salary Range: $18 to $23 per hour (Depending on experience).
