VOICE Therapeutic Solutions is hiring for a Care Manager Extender (CPSS) to join our care management team

Responsibilities

• Share personal story to assist/support  members;
• Perform general outreach, engagement, and follow-up with members;
• Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation);
• Sharing information with the care manager and other members of the care team on the member’s circumstances;
• Support the care manager in assessing and addressing unmet health-related resource needs;
• Other duties as assigned.

Knowledge, Skills and Abilities:

• Strong interpersonal and written/verbal communication skills
• Proficient in Microsoft Office products (such as Word, Excel, Outlook, etc.)
• Good organizational skills to prioritize duties to meet deadlines

Education, Experience and Certifications

· Successful completion of Certified Peer Support Training Program.
