You can now pay your Course Recertification fee online. Please review all the instruction on the Course Recertification page and understand the typical process. You will need:

• An account for this site if you do not already have one. You can create one here (an administrator will approve your account):

• You must be logged into the site to view the order form below and complete the checkout process.
   
• You will need a valid credit card and know the billing address for that card (Visa or MasterCard).
   
• Complete the form below and then proceed through all the checkout steps. You must complete the checkout and payment page for your order to be processed.

You will receive an email receipt upon completing the the checkout steps. Please check your inbox, junk/spam folders. Expect to be contacted by the 40 Hour Course Review Team.
