What We’re Looking For

Trillium Health Resources is transforming public health in North Carolina by managing the health needs of many of the state’s most vulnerable populations. We are currently hiring for a In Reach Extender who is a Peer Support Specialist. The In Reach Extender is responsible for educating members and guardians, throughout the transition process, about how to effectively navigate the physical, behavioral and social  service systems for themselves and about the existence of informal/community resources available to them; and facilitates the member/guardian access to these resources.

On a typical day, you might:

  • Partner with Diversion staff, Transition Coordinators, and Next Step Rehousing Team to facilitate transition planning and progression through the TCL process for those members who desire permanent supportive housing in the community.
  • Collaborate with Trillium Integrated Care Nurse, OT, COTA and Clinician to identify complex care needs and clinical service linkage needs
  • Facilitate community integration, linkage to services and provide education and support for those members who choose not to transition to permanent supportive housing in the community.
  • Utilize person centered planning methods/strategies to gather information, to get to know the members and legally responsible persons to adequately address specific needs of the individual.

Qualifications: 

Education:  High School / GED

Experience: A minimum of one (1) year paid experience in the Health and Human Services field, including experience working with any combination of the BH/SU/IDD population or, paid experience working directly with people experiencing serious and persistent mental illness, severe or prolonged substance use, intellectual or developmental disabilities.  Duties such as experience teaching BH/SU/IDD population students, or working with them in institutional settings, performing general outreach with individuals, coordinating services/appointments, engaging in health promotion activities, linking members to community resources, and supporting a care team would be qualifying experience.

 

 Or, be a person with one (1) year lived experience with a behavioral health condition with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system.

 

Or, be a person with one (1) year lived experience with a behavioral health condition.

 

Or, a parent or guardian of an individual with a behavioral health condition and has at least one (1) year of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (Note that a parent/guardian cannot serve as an extender for their family member).

Relevant experience is defined as experience working with individuals with mental health, substance use and/or I/DD. Experience and knowledge of peer support and experience working with individuals who are transitioning from a facility to independent living

License/Certification: Certified Peer Support Specialist Certification

Must have a valid driver’s license and required to be maintained for this position

Must reside in Trillium’s Northern/Central Catchment Area: - Trillium’s Northern Region servicing these counties: or within 75 miles within/ (Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Nash, Northampton, Pasquotank & Perquimans counties).   Trillium's Central Region servicing these counties: or within 75 miles within/ (Beaufort, Craven, Dare, Hyde, Nash, Pamlico, Pitt, Tyrrell and Washington Counties and surrounding counties). With offices available in Ahoskie or Greenville, NC.
