Peer Support Specialist

Peer Support Specialist employed by  Peter -Elst LLC  DBA Divine Support Edge Healthcare Services shall  facilitate /provide one on one  peer support to  individual served to encourage/inspire recovery  focused    goals. Peer Specialist shall clients in developing their recovery short  and long range goals and action plans. Peer  Specialist  shall drive  their support through  appropriate  Advocacy and  Resource Navigation to link individual served to community resources for  enhanced wellness.

Certified Peer Support Specialist

The Peer Support Specialist provide individualized, recovery-focused support to persons that promotes the development of wellness self-management, personal recovery, natural supports, coping and self-advocacy skills, and the development of independent living skills for housing, employment and full community inclusion. This role can be performed in-person, and/or hybrid.

Warmline Peer Support Specialist

Purpose:

The Warm Line Peer Support Specialist provides pre-crisis support, encouragement, and resourceful information to people who are feeling isolated, managing the symptoms of their mental health and/or substance abuse challenges, and need support or information about resources.

Remote Position

Travel may be required

Hours: 2nd Shift Tuesday - Saturday 4 pm to 12 am

Warmline Peer Support Specialist

Purpose:

The Warm Line Peer Support Specialist provides pre-crisis support, encouragement, and resourceful information to people who are feeling isolated, managing the symptoms of their mental health and/or substance abuse challenges, and need support or information about resources.

Remote Position

Hours: 1st shift Sunday - Thursday 8 am to 4 pm

Duties and Responsibilities:

PSS/Outpatient Therapist

Duties and Responsibilities:

• Provides outpatient counseling

• Interviews clients to evaluate their immediate needs through relevant assessments and gather additional background information for development of a Comprehensive Clinical Assessment, Person Centered Plan and necessary referrals

• Conducts ongoing periodic evaluation and at exit of program

• Maintains up-to-date confidential records on clients and conforms to HIPPA standards • Ensures case documentation is up-to-date and reflective of services provided
