CPSS to serve as a Care Management Extender. Care Management is a service that coordinates both the physical and mental health/substance use care of individuals with Medicaid. This position may be part time or full time depending on the applicants needs and the needs of the agency. Salary is $18 per hour. Peer Support services are structured and scheduled activities for adults aged 18 and older with MH/SA disability. Peer Supports are provided by Peer Support Staff. Peer Support service is an individualized, recovery focused approach that promotes the development of wellness self-management, personal recovery, natural supports, coping skills, and self-advocacy skills and development of independent living skills for housing, employment and full community inclusion.

Certified Peer Support Specialist to work either as a community based worker, working 1:1 with individuals with mental health or substance abuse struggles OR as part of the care management team working to address both the clients mental health and physical health struggles.
